New Federal DOT Chain of Custody Form
On Aug. 30, 2021, a new revised Federal DOT chain of custody form (CCF) must be used for all drug testing services. If old CCFs are used after Aug. 30, a memorandum must be completed by the collector and submitted to the laboratory before testing can occur. This can cause delays in getting results.
If you use Sanford OccMed for your drug testing program and you do all your testing at Sanford OccMed clinics, you do not need to do anything further.
If you use testing sites outside of Sanford OccMed, you will need to order new DOT CCFs and replace any old CCFs in your possession. To order new forms from Sanford OccMed, email us or call (844) 753-6367.
You can also order new forms by following these steps:
- Contact the administrator of your drug testing program before Aug. 30 to order new CCFs. Their contact information is located under the medical review officer (MRO) section of your current CCF.
- Verify that you have the new revised DOT CCF.
- Throw away any old forms.
- Bring the new CCFs to Sanford OccMed at the next time of service.
